Participation Agreement

In consideration of being allowed to participate, each participant and their legal representative, waive all claims for injury, accident or loss of any kind and hereby release DynaSports Group (DSG), all event operators, camp sites, sponsoring organizations, and their employees, members and representatives from any claims or liability. As further consideration to participation, each participant and their legal representative consents to DSG and its event operator taking photographs and video of participants at event sites, and DSG and its event operator retain the right to use any such video or photographs for publicity and advertising. This Agreement shall apply to the DSG Basketball Camp, held from September 1, 2011 through August 31, 2012.

By submitting this Entry, you are agreeing on behalf of the player named, to participate in and pay the entry fee for the DSG Basketball Camp. To withdraw from the camp, you must advise our office by email at NextTraining@DynaSportsGroup.com.

To complete your registration, you must:

Make payment online and click 'submit' on the next page, or select the mail option, click 'submit' and mail entry fee payment in the form of Certified Check or Money Order to DynaSports Group, P.O. Box 271595, Flower Mound, TX 75027-1595. You may also pay with credit/debit card by calling 972-355-3788 x121. The entry fee must be received within 5 business days and by the registration deadline to hold your entry in the camp. If the entry fee is not received within this time period, your entry, at the option of DSG, may be deemed to be a withdrawal. Upon submission of your entry you will receive a confirmation email with a Participation Agreement attached which will need to be signed and turned in at camp check-in.
